Wiki Colon screening vs Surveillance

Hi all - I need clarification please...

If a 48yo Aetna pnt has a colonoscopy w/ polyps removed, and it is suggested he return in 3 years for follow up... does the primary DX need to be Z86.010 or can it be Z12.11 since this is his first colon at 50+? I'm thinking it needs to be Z86.010 but need confirmation.

With a history of colon polyps and it being noted that follow up is needed, I would not code it as a screening.
