Difference of opinions on this scenario: Screening Z12.11 turns diagnostic with a polyp in sigmoid snared 45385 and polyp in ascending biopsied 45380
Commercial insurance: 45385/33 z12.11 as primary and D12.5 secondary , line two would be 45380-59 Z12.11 as primary and D12.2 secondary
So , is anyone putting the Z12.11 on the second line for the 45380? We always were taught to link the reason for the procedure in the primary slot.
Does anyone know for sure about coding in this scenario? It seems to me if Z12.11 was not primary for 45380 then the screening benefits would
not be applies. Any info will be helpful

~ I would use the D12.2 for 45380 , no need to use the Z12.11 . You have established that it was a screening on the 45385 already to let the payer know that it was for a screening that turned diagnostic.
