Could I get help in dx for this path report.
Colon @ 20cm, Polyp, Snare
Tubulovillous Adenoma with high grade Dysplasia.
Cauterized normal colonic Mucosa is Present at the base of the polyp.

Comment:
Multiple additional levels were obtained and show that in addition to the high grade dysplasia, focal carcinomatous changes are present. However these malignant changes appear confined to the polyp.

Based on the comment of additional findings after additional levels, I would code this as a carcinoma of the colon: 153.9
