What are the appropriate code levels for an established pt with common cold with cough, with Rx given? without Rx given?
How about upper respiratory condition with Rx? without Rx?
99212, 99213 or 99214?

Code choices have to be based on documentation of required elements per the CPT code description, not on diagnosis. However, in an otherwise healthy patient with no comorbidities or complications, most payers would expect an established patient visit for these examples to not exceed 99213.
 
Agreed. What if pt has diabetes and Hypertension as comorbidities with URI?
COPD with URI would that still be a 99213?
