Poorly controlled DM, on Lantus, has cataract (unspecified) but is going to Ophthalmology for a diabetic eye exam: E11.36, E11.65, Z79.4? And shouldn't we code for the unspecified Cataract?

No, E11.36 is sufficient for the cataract. The unspecified code is not accurate, as you know the cataract is a Diabetic cataract.
