Report only one or the other. 20610 is without and 20611 is the same procedure but with guidance
The only scenario I can think of to bill both codes is if they are two separate injections. Our providers sometimes do multiple joint injections at the same encounter (hip/knee or hip/shoulder, etc...). One of the injections may require U/S guidance & the other not.
