Wiki Coding results of xrays on Outpatient charts

Can anyone help me find the coding guidelines that mention coding signed xray and other diagnostic results? There is a guideline that states the result of xrays, verified and signed by a provider should be the first listed code and then other signs and symptoms.

I've seen this somewhere but I cannot locate the guildline.
