hi...i'm finding a lot the imaging-- CT MRI denying for medical necessity even after appeal. Does anyone know if a secondary diagnosis would help when billing the original claim?

More information is needed. Any diagnosis listed on the claim must be supported by the medical record. Can you provide an example of a denial that you received? Are you coding based on signs/symptoms or the radiology report? If you can provide an example of a denial you received, that would be great.
