Where would I find information on the definition for Monitor, Evaluate, Assess, Treat.

If a dx is listed under Current Problems but not mentioned again in the exam or the assessment, is this considered "monitoring" the condition and can it be coded?

I would not, just to list a dx and then not discuss it again in any fashion? I say no code.
