I have a coding question that is puzzling our office. We do abortions and wanted advice on coding the pre abortion visit. A patient will come in and have the labs done with an ultrasound and have options counseling. At this point she isn't having the abortion. She will come in the next day for the actual procedure or medications for the medical abortion. Do we code the 635.90. legally induced abortion and V22.1 pregnancy as that is the reason she is there, or do we just use the pregnancy code as she isn't actually having the abortion that day?
We have done it both ways but are finding that sometimes the private insurance carriers will deny the V22.1 as a well baby visit.

What about a counseling code, such as v65.49. You are doing counseling of options and educating the patient on the abortion process, correct?

I don't have experience billing abortions but this seems logical.

We bill the confirmation of pregnancy visit with 626.0 and v72.42.

Do not use the 635.9x diagnosis until the day of the abortion.
