How would I code this?

A patient who goes under for nail debridement.
Patient does not meet any class findings or meet any ICD10 codes to support medical necessity, (MAC Novitas JL) so I'm thinking this may not be covered.

Patient is a x year-old with Down syndrome. Is unable to tolerate routine nail debridement in an office setting. Patient does have his nails debrided every 12 weeks under monitored anesthesia care. His diagnosis is Onychomycosis, bilateral toenails x 10. The procedure is debridement of elongated toenails 1-5, bilaterally.

Operative Technique: Patient was brought into the operating room and remained on bed in a supine position. Patient was placed under MAC. Patient's toenails 1-5 bilaterally were noted to be thickened, elongated, discolored, and dystrophic with subungual debris. A nail nipper was used to perform sharp debridement of all of the mycotic nails x 10. The nails were smoothed within the OR with a nail nipper and trimmed sufficiently. Patient tolerated the debridement and anesthesia well. Patient was transferred to the PACU with stable vital signs. After period of postoperative monitoring he will be discharged.
 
Oh goodness, I am so sorry to have to tell you this... The appropriate cpt code will be 11721 and the appropriate dx will be pain in toes, that is all the doctor will get paid for. You can try adding modifier 22 and add the downs syndrome dx and supporting data in box 19/loop 2300, but I don't think this will get you paid any more than the approximately $45 that is allowed. It is the "good deed" for the day. Might consider removal of the nails if this continues to be a problem for the patient, not ideal, but may be best for this particular case.
