How to Code Multiple chemotherapy drugs

Hi,
Can someone tell me the best way to code multiple chemotherapy drugs since there are only 3 codes for chemo infusion given more than 15 mins; 96413 for 1st hr, 96415 add on and 96417 for concurrent.

Example: Patient recieve 3 chemo drugs each lasted 2 hrs and each started and ended at different time .

96413
96415
96417x4
or

96413
96415
96415
96415
96415
96417

or
96413
96415
96417

Thanks

coding drugs started at concurrent time

Thanks a lot @ mjm41795. It was lot help.

One more question. If in same scenario, 2nd drug was started same time as 1st drug was stop and 3rd drug was started same time the 2nd drug was stop than would it be correct to report just
96413
96415
96417

Thanks

If I interpreted your question correctly, you have 3 drugs given subsequently over 2 hours each?

example: Drug A started at 9am, stopped at 11am
Drug B started at 11am, stopped at 1pm
Drug C started at 1pm, stopped at 3pm

Drug A is coded with 96413 (9-10am) and 96415 (10-11am)
Drug B is coded with 96417 (11am -12pm) and 96415 (12-1pm)
Drug C is coded with 96417 (1-2pm) and 96415 (2-3pm)

Codes:
96413x1
96415x3
96417x2

Thanks. Yes you guessed it correct. Since they were started at sametime when other was stoped; will it still be appropriate to code all three.

example: Drug A started at 9am, stopped at 11am
Drug B started at 11am, stopped at 1pm
Drug C started at 1pm, stopped at 3pm
