I recently came across the question on coding old MIs. Doctor documents the patient has coronary artery disease with stents, history of unstable angina and the patient has had a MI. He brings into his visit diagnosis of old MI and on medication but doesn't state when the MI nor unable to find the MI in the chart, (cardiologist hasn't mention an old MI). The physician just started seeing this patient in 2008 but stated that the unstable angina was found in 2000 when the patient went to the ER but it still doesn't mention a MI. Can you still code the old MI without stating when the MI occured and this is the only documentation in the office visit note? We are trying to get our physicians ready for any Medicare audits.
