Need some coding assistance.

PT had FX to shoulder region & was treated in OR in 05/2010.

PT returns to clinic for followup visit on 07/23/2010. 2 weeks prior to this visit patient fell in bathtub & now has A/C seperation DX- 840.0.

We're still within 90 surgical global period-however re-injury due to accident patient had in bathtub-should I code this out as both a followup visit 99024 w/V54.89 as well as E&M o.v. charge of 99212 w/modifier 79 attached & DX 840.0?

Yes, I would code this as a new injury w/ an office visit 99212 with 24 modifier with the dx 840.0 which is the new injury.
You cannot use 79 modifier for E&M codes only procedure codes.
