If a physician requests a pathologist to go back to an old case and re-evaluate it or clarify it requiring them to pull the case from the archive, review the report and slides and issue an additional path report as an addendum, is the pathologist able to bill for it? If they already billed with CPT codes 88300-88309, which codes would they use for the re-evaluation?
