Can anyone help me with coding this procedure?
Incision was made after she was prepped and draped in normal sterile fashion. Careful incision down to the paraspinals, allowed us to identify the transverse processes on the patient's left side at L4-5 and S1. Pedicle screws were then placed. We did perform a massive decompression centrally into the nerve roots of L4, L5, and S1 both the right and left. We then placed in the pedicle screws at L4-5 and S1 on the right. A transverse connector was placed and posterior lumbar interbody fusion was performed with PEEK cages packed with bone graft and the interspaces were packed with autograft and allograft along with posterolateral bone grafting on the right and left sides. We were then able to perfom placement of 2 medium Hemovac wounds. 1g of vancomycin was placed in powdered form. We were then able to close the wound with 0 vicryl, 2-0 Vicryl, and a running Prolene stitch. The patient had Dermabond placed and sterile dressing was applied.
Operation:
1. Decompressive for spinal stenosis
2. Identification of L4-L5 and S1 nerve roots, right and left.
3. Placement of pedicle screws at L5, L4, and S1
4. Posterolateral bone grafting
5. Anterior lumbar interbody bone grafting, consituting 360 degree fusion.
6. Placement of prosthetic cages with bone graft in the interbody spaces at L4-5 and L5-S1, autograft harvest.
Allograft Used: Bacterin bone graft allograft. Spine Wand pedicle screw system was used as well there are interbody spacers.
