Wiki Coding for Plan of Care for Physical Therapy?

My Ortho doc has created Plan of Care for patient. Is there a code for that? What I've found is G0180 or G0179. Would that be correct?

He thinks it bills for around $15, but G0180 bills for $122 and G0179 for $94.

Does anyone have suggestions? thoughts? Knowledge?

Is this for home care? Those 2 codes only work for Medicare. Have to use 99374 (supervision of patient under care of home health agency) for all other insurances.
