Wiki Coding for IUD due to a recent Expel IUD

Hello,
do anyone know how to code for an insertion of an IUD when a recent IUD had fallen out (about two weeks later)?
Is there a modifier to let the payor know this is due to complication?

The IUD codes are pretty straight forward with removal and insertion.

Your diagnosis codes should support the complication with a T8X.XXX and also a Z30.43X

We utilize the modifier 76, since none of the other modifiers apply (such as 58 as there are no global days). We submit with medical records and a letter explaining the reason for the additional service.
