For an excision which size do I go by to get the correct code, is it the post op or the excision diameter:
Please Help.
Pre op size 1.1cm by 1.2cm
Post op size 1.4cm by 3.9cm
Excision diameter 1.4cm
Appropriate margin 0.2cm
Final repair length was 3.9cm
 
Excisions

To report excisions (CPT codes 11400-11446 and 11600-11646) the size of the lesion diameter is used for proper code selection. Also, note that lesions are measured by the diameter, while repairs are measured by the length, width & depth of the wound. In addition, it is important to code for the lesion diameter prior to pathology because the specimen may shrink and results would not reflect the true size of the lesion excised.
