We see several patients with chronic pain on long term opioid medications. I believe that we would code the chronic pain g894 for the first code and then also code with long term chronic opioid Z79891. The patients have signed pain contracts in the chart. The diagnosis listed for visit is the chronic pain. Should the office note also say long term meds or the fact that the pain contract is in the chart suffice.?
