We had a patient (71 yr. old female, insurance is Medicare) come in to have a physical done so she could have clearance for left foot surgery. The provider coded it as a preventive service 99397 but the documentation clearly states it is a physical for surgical clearance plus there were other diagnostic procedures done. I would like to code it as an evaluation and management office visit along with the other codes for labs, xray, etc.. Does anyone have any thoughts on this?
