Wiki Coding excision of gouty tophi of great toe

I am having a problem coding an excision of gouty tophi of the great toe. The codes I was leaning towards are 28092 and11420. The reason Im not sure about the 11420 is a tophi is not really a lesion. The second part of the question is can these codes be used if the procedure took place in an office setting? I am new to the MBC field and podiatry is not the speciality I am used to coding for. Any suggestions? Thoughts?
