Wiki coding clarification

The family practitioner I bill for documented "patient here for preventive care per work request".

What ICD-9 and CPT would you use to bill this claim?

I am asking to see the responses of others, so that I may show the physician that I coded this correctly.

Use a V70 dx code and the appropriate preventive E&M code by age of patient.
