Hello, Need an opinion...

DM type 2 with renal manifistations not stated as uncontrolled
Hypertension, benign
CKD stage 3

Dr coded: 250.40
401.1
585.3

I coded: 403.10
585.3
250.00 Since Dr didn't state the CKD was due to DM

Any points would be most welcome

I would code it as 250.40, 583.81(although the Dr is not stating it's due to the CKD. He is stating there is a renal problem), 403.10 & 585.3.

Let's see what anyone else thinks

what he didn't say

He said the pt had DM w renal maniestations. What he didn't say is what KIND of renal manifestations- you correctly can't jump to CKD unless he calls it that. You also can't just pick diabetic nephropathy as he didn't state that. And if he calls it anything BESIDES CKD make sure you check your HTN table to ensure that code entitles you to code it as hypertension with chronic kidney disease. At my place of business we are allowed to take DM WITH xxxxx as showing causation. Since you have to query about what manifestation you can ask him to state it as DIABETIC whatever.
