This is a case of right eyelid ptosis, right facial numbness and mild right hemiparesis in a 38 year old female after taking Tosympra and four days post Botox injections for chronic migraine. She also recently has been experiencing chest pain and palpitations. Based on her presentation, there is concern for an acute ischemic brainstem stroke. She is almost 24 hour from onset, therefore out of the window for TPA.

Patient was referred to the emergency room department for acute evaluation. She is able to drive herself at this time. I contacted the emergency physician at Centennial Medical to coordinate care and recommendations for evaluation. We discussed that Botox could cause of lid ptosis but should not cause eye pain, blurry vision, right facial and arm numbness and weakness. Therefore discussed the need for emergency evaluation. In this case, What modifier do i need to use for the reimbursement?
Please help me out in this case? TIA.
