Wiki Coding a recent condition

Hello,

When a provider documents "recent pneumonia" or "recent sepsis" or "recent COVID-19", are these conditions coded as active conditions or "history of" conditions ?

The provider should be able to tell you this if you ask them. I would think "recent" would mean active, but I would check with the provider to make sure that's what they mean if you are able to send them an inquiry.
