Does any one have any information How to bill a patient whom being seen to renew of there CDL , PA'S in the office is certified to do these exams but have been getting conflicting information. Some say bill E/m code with dx V703 OR V705, to the insurance whom may deny and then bill member, Others say bill patient at visit with E/M code and have member seek reimbursement from Employer.

we use 99455 or 99456 with V70.3.
we also have patient sign ABN letting them know exam may not be covered by insurance as it is considered an administrative exam by most companies.

We also charge a 99455 and we require payment at hte time of service. DX is V70.3
