I am in the process of completing Code-A-rounds for Dermatology. I noticed on Round 2, note 4 there seems to be a discrepancy, I believe. Operation: states intermediate defect closure measuring 15 mm and 14 mm (there are 2 lesions upper back). The CPT code that the Code-A-round completed as correct was 12031 (2.5 cm or less). I believe it is 12032 (2.6cm to 7.5cm). Would you not combine the lengths 15mm (1.5cm) and 14mm (1.4 cm) due to them being in the same anatomical site? If this is true, the correct code would be 12032, because 1.4 and 1.5= 2.9cm. Also.... if there are 2 lesions, would you not need a multiplier/unit of 2? The Code-A-round completed note as correct with just a modifier of 51 on 11401 without the multiplier/units with 2. Please advise. I am confused. Thank you. Cindy Roberts, CPC, CPCD
 
The repairs to the same site, same classification are added together. So I agree with you on the 12032. No modifier is needed for that.

For the excisions, you do need a modifier 51 and a "x2" added to the 11402 (not 11401) based on the measurements.
 
You code the lesions excisions separately and not with a unit of 2, you should list each one as a 11402, then you add the lengths of the repairs together with just 1 unit.
