Wiki CMS Allowable Facility Fee

This may be a silly question to many of you (so I apologize in advance), but can someone remind me which fee you would use from CMS if you are billing Part B for a physician? Example: Physician codes 99291...do you use Facility or Non-Facility allowable?

It depends on the place of service code you used on your claim. If the service was provided in a hospital or an ASC, then you would use facility. If it was performed in the physician's office, you would use non-facility.
