If a physician uses the term "stroke" as a diagnosis for an office visit, would you use 434.91 or the history code? Is the 434.91 considered to be for an acute stroke or can this code be used at will?

434.91 is an acute code, is there anything more in the note? what is the reason for the encounter, in what context is the word stroke used?
