Good Morning- I have been wracking my brain trying to figure out the appropriate way to code a visit that was for an evaluation prior to admittance to a respite facility. The provider discussed all of the patients history with the family and then added all of the conditions the facility should be monitoring while the patient is in the facility for 30 days.
The provider did not document time spent with the family going over any of the details, and there is no plan of care for any of the conditions as far as medication management, just that statement that those are what the facility should be monitoring.
Any thoughts or suggestions on this would be greatly appreciated!
