Wiki Clarification regarding CKD Stage

Which stage of CKD should be coded when two different stages documented?
For e.g: N18.1 and N18.3 both documented in MR then which level should be coded??.. Any help is appreciated.

You will need to query the provider to get clarification of the correct stage.

Any other suggestions for choosing stage of CKD? like when CKD Stage 2-3 documented we will be coding CKD stage 2.
and when both CKD and ESRD documented will be going for ESRD(as per ICD 10 guidelines). likewise any suggestion for N18.1 and N18.3 when both documented???
