Which claim form is used for Hospital Based Outpatient Clinic? Is it going to be Ub04 or HCFA-1500 or both? If anyone knows, please help.

Are you a physician specialty clinic that has an outpatient clinic in the hospital setting? CMS1500. If you are an out patient clinic that is associated with the hospital that will be charging a facility fee, most likely both, CMS1500 for physician charges, UB for facility charges.
