If a physician documents CKD stage lll/IV is there a coding clinic stating which one to use?

You cant be both at the same time. Physician will need to be queried. There is no overlap on the GFR scale.

I have usually coded the lower stage. Look at the GFR score and see where pt stands. there are other factors too that determines CKD stage. Yes, query your physician.
