Good morning everyone,

Patient has CKD along with HTN and DM.Doctor does not write CKD due to DM/HTN. As per the Physician he/she does not know the cause for CKD.

As per the coding guidelines it assume 403.9X along with 585.9

I heard from someone that I can code 250.4X (DM with renal complication) along with 403 and 585 codes.

Is there any guidelines that you can share with me. I would like to do it right without any future problems (Audit issues)

The coding guidelines you quoted in relation to the CKD/HTN is correct in that there is an assumed relationship with or without the doctor saying so or making that connection. You would therefore have to code them together. In respect of the DM, the doctor would specifically have to indicate that there is a relationship between the DM and CKD. In the absence of any documentation from the doctor to indicate the link, you would have to code it as a stand alone code.

It would be a good idea to query the doctor, just to satifsy yourself that there is/isn't a connection between the two diseases.
