To bill CCM, your practice would need to be providing an ongoing management of patients' chronic conditions. The patient must give a written consent to receive this service and the codes are billed monthly based on the time the practice has spent in managing their care. These services are not a component of E/M services and can be billed separately since they represent the ongoing work of the provider and office staff in monitoring the patient and coordinating care outside of the office setting and throughout the month.
