I would use 724.5. ( Back Pain ) 338.29 I feel is not specific enough whereas 724 gives the site of the pain.

Actually guidelines direct you to code both. The site of the pain and the 338 code for the acute or chronic status. If the visit is for pain management then you sequence the 338 code first
