I'm wondering if anyone can assist me in how you would code the following:

Chronic anticoagulation secondary to atrial fibrillation and aortic valve replacement

I got 427.31, V58.61, V43.3

Here's the HPI:

Patient presents today for anti-coagulation followup & PT/INR for chronic anticoagulation for atrial fibrillation and aortic valve replacement. The patient has been using their coumadin as directed which is: coumadin to 5mg 1.5 tabs daily except Tuesday & Thursday take 2 tabs daily. The patients last pt/inr was subtherapeutic at 1.6. The patient denies any headache, epistaxis or bleeding gums, cest pain or discomfort, heart palpitations, cold hands/feet, dyspnea, cough, hemoptysis, hematemesis, hematochezia, hematuria, shortness of beath, dizziness, or lightheadedness.
