If I am coding from the operative report and the post op DX is Spondylosis without myelopathy or radiculopathy, lumbar region, the Dr. injected L4-L5, L5-S1, should I also be coding spondylosis without myelopathy or radiculopathy for the lumbosacral region even if not documented as the post op dx?
