The patient was then placed in the left lateral
decubitus position. The skin of the right side of the chest was prepped and
draped in the usual sterile manner. A 2 cm skin incision was then performed.
The fascia and subcutaneous tissue were entered with electrocautery obtaining
hemostasis simultaneously. The right lower lobe was identified and a portion
was resected with an Endo-GIA stapler. Total hemostasis was ascertained. Lack
of air leak was demonstrated. Intercostal nerve cryolysis was performed at the
level of incision, one level above and one level below. A pain pump was
inserted in the usual manner. A chest tube was inserted into the pleural
cavity. Ventilation was given. No air leak present the system was removed.
The fascia and subcutaneous tissue were approximated with 2-0 Vicryl and the
skin with 4-0 Vicryl subcuticular. The patient tolerated procedure well and was
transferred to recovery room, extubated in stable condition.
