Wiki chemo and vitamin infusion on same day

I'm looking for some suggestions for the following scenario:
- Naturopathic Doctor (ND) orders Vitamin infusion to be administered on same day as chemo because there is a better patient response.
- Medical Oncologist (MD) orders chemo.
- Patient receives pre-med, followed by Vitamin infusion, followed by chemo
- CPT 96367 is for the additional sequential infusion, new drug or substance, up to 1 hour
- CPT 96365 is for the initial infusion up to 1 hour
- CPT 96413 is for initial chemo infusion up to 1 hour
- Notes state "report 96367 to identify therapeutic/prophylactic/diagnostic drug infusion if administered as a secondary or subsequent service in association with 96413 through the same IV access"
- no NCD or LCD available
- my understanding is that only one initial code (96365 or 96413) can be billed on the same day
- How can I bill this for 2 different providers at the same infusion center on the same day and comply with the 'only one initial code per day' guideline?
