Wiki Charging for "H&P's" a week to10 days prior to surgery??

A question was brought up to me "Can we charge for History and Physicals as a clearance for a planned future surgery?" We are reviewing labs, imaging, Heart doctor notes, etc. to make sure the patient is okay to have surgery. Surgery is a week to 10 days later. Has anyone ran into this before or what are you doing in for your H&P's? Are you charging for an office visit? Or bundling it into the surgery package?
 
Once the decision for surgery has been made, the H&P work done by the surgeon prior to surgery is included in the global surgical package.
There are nuances and sometimes the actual decision for surgery is made at that visit, which would make it billable.
