Wiki Change of Assistant Surgeons

Hi, I need some advice about assistant surgeons. One of my surgeons had an assistant surgeon who was not able to stay past a certain time during a surgery and another assistant replaced him about half way through the surgery.

How do the assistants bill this? It's a new one on me!
