per the doctors operative report.......The target point of the lesion was at the center of the trapezoid seen of the vertebral body on lateral view on the fluoroscope.

Does this indicate a true RFA NERVE ABLATION. He made no mention of the target nerve, facet joint, medial branches....nothing like that. Does this op note make sense and qualify for a Cervical RFA 64633 ?

Maybe look at back at past notes where they performed the cervical medial branch block and see which levels/medial branches were treated then review the order and note for the decision to proceed with RFA procedure and confirm what was scheduled. Look at the consent for the procedure and what levels were consented for. The immediate brief postoperative note and finally if the procedure note is unclear then request the physician to specifically address which facet joint nerves were treated with the procedure.
