Codes billed and rejected: due to Procedure J0696 must be billed with valid NDC data. Required elements are the valid (11 digit) NDC number without spaces or hyphens, the unit of measure, and units dispensed.

I double checked the NDC code and it is correct. I am thinking it is the units that is incorrect but not 100%.

J0696 Ceftriaxone Sodium 1 gram per single dose bottle 2 units were billed
96372 THER/PROPH/DIAG INJ SC/IM 1 unit was billed

In this scenario the patient came in and the doctor ordered this shot.

I suppose I am a little confused about the Units and cc's.

The shot includes Lidocaine which when added it makes the shot 2.5cc's.


So my question is should the CPT code J0696 have 2 units or 1?

Any help will be greatly appreciated.

Think about the NDC units in terms of the bottle. If 2 whole bottles were used, then bill the NDC with 2 UN. The quantity for the J code is determined by it's descriptor- J0696 is per 250 mg. So if 1 gram (1000 mg) was given, that would be J0696 x4.
