My doctor removed an ICL (Implantable Contact Lens) during cataract surgery. Would it be appropriate to bill 66984 and 65920 (removal of implanted material, anterior segment)?

If the physician placed a new lens, then your code would be 66986; otherwise, I believe from your description the code would be 65920 only.

My surgeon removed an ICL, then removed the cataract, then placed an IOL. I originally coded it as a 65920 plus the 66984. There are no bundling issues with CCI, so this is how I left the coding. It is now in a code review status, but not sure what the outcome will be. Thanks for the input.
 
I have a similar case that our doctor is wanting to schedule at our office. My front office staff was trying to schedule it in our system and the doctors office just wrote ICL removal and cataract surgery. They called me asking which codes to use when scheduling. At first I said 66984 + 65920 then 66986. I'm torn. Then I wonder if the patients commercial insurance would even pay for the ICL removal portion. I question 65920 because under the lay description of this code it specifically states the lens is removed and not replaced. Can you tell me how your case worked out?
 
Our case came down to the ICL removal and a catarct removal. It was coded and paid as was stated above. The 65920 plus the 66984 were both paid. Because there was no new placement of ICL, we steered clear of 66986. After checking the EOB, it is clear that the payer accepted our coding. I hope this answers your question.
