Wiki Cast Repair

I work for an pediatric orthopaedic group.
My question is: when a child comes in to have the cast repaired, if the whole cast is not replaced, just repaired in the place it has worn through, how do I code for that?

If it is a nurse doing the repair, a 99211 could be used based on documentation and if the doctor is present in the building and code for the supplies used.
