Hello-
We are getting denials on C1725, C1874, and C1887 for no authorization. We have not been getting authorizations on these codes as they are for supplies. All of them have Rev code 0278 which does not meet the FDA's criteria of an implant that must stay in the body for 30+ days. We use C9600 with these codes. I am wondering if changing the Rev code to 0272 would be more appropriate? This is for facility billing. Any guidance would be greatly appreciated!
