I am new to this kind of billing. Do you build a code to associate with the PMPM payment and create claims for each patient and post each payment individually?

Yes. See how your software handles it. Remember that not everything that happens is capitated; you will have to check your contract. For instance, in primary care, an EKG may or may not be capitated.
