Can I bill a 77003 with a 59 modifier along with facet, transforaminal, epidural injections?

Some insurance companies are reimbursing for it while others aren't. Should I stop or is this o.k.?

You should not separately report imaging guidance, specifically fluoroscopy or CT as the CPT codes for the injections includes imaging since evidence based medicine studies indicate that injections without imaging guidance are not very effective and should not be performed without guidance. Check your CPT codes in the AMA CPT book and it will indicate this.
